FIELD: medicine; oncology.
SUBSTANCE: invention can be used to improve long-term results of treatment of patients with central lung cancer after performing conditionally radical surgical interventions. In patients with non-small cell lung cancer with morphologically confirmed R0, N0, T1-2, with a distance from the tumor to the resection line of not more than 3 mm, 1 month after surgery, the 1st course of photodynamic therapy is carried out according to the following method: “Fotoran” photosensitizer is injected IV dropwise at a dose of 2 mg/kg body weight per 200 ml of physiological solution over 30 minutes, 3.5 hours before the intended irradiation; within 24 hours after administration of the photosensitizer, a light-protective regime is recommended for the prevention of skin burns; protection of the cornea of the eyes — sunglasses, using an Actus device, 662 nm, a light guide with a microlens, 3.5 hours after the IV administration of the photosensitizer, a PDT session is performed from 4 points on the area of the bronchial stump until Ws = 200 J/cm2 in each point, at laser P 1,000 mW. 2 months after surgery the 2nd course of photodynamic therapy is performed, and 5 months after thoracic surgery the 3rd course of photodynamic therapy is performed. In patients with N1, additional 4 courses of adjuvant chemotherapy are performed according to schemes in line with clinical recommendations — the 1st course is performed after surgery, the 2nd course is performed between the 1st and 2nd courses of photodynamic therapy, the 3rd and 4th course of polychemotherapy is performed between the 2nd and 3rd course of photodynamic therapy.
EFFECT: invention can significantly reduce the frequency of local relapses, increase the duration of overall and relapse-free survival while maintaining the quality of life of patients.
