FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to oncology, and can be used for combined treatment of patients with locally advanced gastric cancer complicated by subcompensated tumour stenosis. Method involves photodynamic therapy and polychemotherapy according to FLOT or XELOX regimens. Before the first and third cycles of polychemotherapy, three days before the beginning of the cycle, a session of endoscopic photodynamic therapy is performed. For this purpose, the photosensitizer Photoran E6 in dose of 2 mg/kg of body weight, dissolved in 200 ml of physiological solution, is administered to the patient intravenously for 30 minutes, 3 hours later, the tumour is exposed to dose of 200 J/cm2 with the help of a “СИЛМА-01” diffusion-type quartz light guide with diameter of 1.6 mm and a “ЛАХТА-МИЛОН” apparatus with the following parameters: wavelength 662 nm ± 1% in pulse mode with power 900 mW, pulse duration is 150 ms with interval of 90 ms. Tumour area is calculated by multispiral computed tomography of the abdomen with intravenous contrast enhancement performed at the stage of the primary examination of the patient.
EFFECT: use of the invention enables to achieve an increase in the effectiveness of the treatment of locally advanced gastric cancer complicated by subcompensated tumour stenosis by improving the overall and recurrence-free survival rate, improved tolerance of drug treatment, higher tumour response to neoadjuvate drug treatment and radicality of surgical treatment by complex effect on tumour.
