FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to plastic, maxillofacial ENT surgery. Subperichondrial and subperiosteal detachment of mucocutaneous flap is followed by sampling of septal cartilage. Then the cartilage is used to form transplants of rectangular shape with thickness of 1 mm—type-grafts and a transplant in the form of a shield. Between the medial pedicles of the alar cartilages, a double spacer columellar graft is fixed. Type-grafts are sutured to the domes of the tip of the nose in a certain sequence. First graft is laid to the descending part of the formed domes at angle of 50°, guiding it ventro-caudally and fixing the edges with single interrupted sutures. Further, the next type-graft is laid parallel to the first one and fixed to the first one with single interrupted sutures. In the central part, the type grafts are fixed to each other with a blanket suture. Shield-shaped graft is laid between the medial pedicles of the wing cartilages and anchored to a spacer columellar graft. Mucocutaneous flap is placed in place. Obtained aesthetic result is visually assessed and sutured. Intranasal splints are inserted into the nasal cavity and fixed with one interrupted suture. Splints are removed on an outpatient basis on the third day. Strip bandage is applied on the nose. External thermoplastic nasal splint is fixed and removed on an outpatient basis on 14th day.

EFFECT: method enables elongation of the end portion of the nose with the formation of an optimal projection in the patients with deformities of the osteochondral portion of the nose.
