SUBSTANCE: invention relates to plastic and maxillofacial surgery, otorhinolaryngology, and can be used in treating patients with congenital and acquired deformities of osteochondral part of nose. Performing subperichondrial and subperiosteal detachment of soft tissues from lower and upper lateral cartilages to root of nose. Mucocutaneous flap is formed. Dorsum and cartilages of an end portion of the nose are remodeled. Then mucocutaneous flap is placed in place and precartilaginous incisions in the area of lateral and medial crus in one nasal passage are sutured with simple interrupted sutures. In the second nasal passage, a precartilaginous incision is closed in the area of the lateral crus. Further, from plates of pre-collected septal cartilage with the help of Cottle endotome, conglomerates of crushed cartilage are formed with hammer blows. Formed conglomerates of crushed cartilage are laid through a precartilaginous incision in the region of the medial pedicle using forceps into a skin pocket of columella and in the area of the sub-tip zone in the required volume. Reverse side of the forceps is used to evenly distribute the cartilage by tapping on the skin. Incision line is sutured in the region of the medial pedicle. Intranasal splints are inserted into the nasal cavity and fixed with one interrupted suture. Strip bandage is applied on the nose. External thermoplastic nasal splint is fixed.

EFFECT: method enables reconstructing the anatomy of the terminal portion of the nose, forming the aesthetic contour of the columella in treating the patients with congenital and acquired deformities of the osteochondral portion of the nose.
