SUBSTANCE: invention refers to medicine, particularly to plastic, maxillofacial and ENT surgery. Performing subperichondrial-subperiostal detachment of soft tissues to complete skeletonization of bone and cartilage pyramids of external nose. Piezoelectric apparatus is used to perform complete lateral and transverse osteotomies and incomplete, notched, paramedial osteotomies of the nasal bones with mobilization of the bone pyramid and preservation of the intact key zone of the nasal dorsum. After excision of a fragment of cartilaginous and bone parts of nasal septum in the form of a strip according to the required level of lowering of the back, the lateral nasal bones are reduced, the bone pyramid is narrowed in the upper and lower parts and the lowering of the back of the nose. Further, an end portion of the nose is formed according to anatomical landmarks. Mucocutaneous flap is placed in place. Obtained aesthetic result is visually assessed and closed. Intranasal splints are inserted into the nasal cavity and fixed with one interrupted suture, which are removed on an outpatient basis on the third day. Bandage of strips is applied on the nose, an external thermoplastic nasal splint is fixed, which is removed on an outpatient basis on 14th day.

EFFECT: method enables providing additional effective and stable narrowing in the upper part of the bony pyramid of the nose, preventing aesthetic complications in the form of insufficient correction of the width of the middle one-third of the nose.
