FIELD: medicine; maxillofacial surgery; neurology and rehabilitation.
SUBSTANCE: on the 4th day after surgery, a preliminary clinical and functional examination of patients is carried out, the said examinations include palpation and electromyography to assess the condition of the masticatory muscles and the severity of pain in them. The injection areas are marked as follows: three points are marked in the area of the right and left masticatory muscles and three points are marked in the area of the right and left temporal muscles. Then, one point is marked in the area of the right and left lateral pterygoid muscles and one point in the area of the right and left medial pterygoid muscles. Then botulinum neuroprotein type A is restored with 2 ml of a sterile solution of 0.9% NaCl. After antiseptic treatment of the injection zones, the medicinal product is injected in a sitting position in one session into the masticatory muscles, 3–5 units of the medicinal product per point, into the temporal muscles, 3–5 units of the medicinal product per point, into the lateral and medial pterygoid muscles — 10 units at one point under electromyographic control using a needle electrode. Before administering the medicinal product, an aspiration test is performed. Then, after removing the splinting structures, the patients are prescribed myogymnastic exercises in the form of opening/closing the mouth, lateral movements and extension of the lower jaw, performed 3–5 times a day during the rehabilitation period.
EFFECT: method makes it possible to increase the efficiency of rehabilitation of patients with a fracture of the lower jaw and long-term immobilization of the jaws, to increase efficiency and reduce the time of rehabilitation, starting from the early postoperative period.
