FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to plastic, maxillofacial and ENT surgery. Subperichondrium and subperiosteal detachment of mucocutaneous flap is performed from lower and upper lateral cartilages to nasal root. Piezoelectric device is used to remove an upper bone portion of the nasal dorsum. Lateral, incomplete transverse and complete paramedial osteotomies of nasal bones are performed. Then, 3 holes are formed in the right and left nasal bones with a cone-shaped drill bit of a piezoelectric apparatus for drilling bone tissue. Then, using a scalpel, a fragment of the dorsal part of the nasal septum is excised according to the required level of dorsal lowering. Further, the bone and cartilage parts are brought together, fixed and stabilized with a double "corset" suture using a non-absorbable monofilament synthetic suture 4/0. Then, an end portion of the nose is formed according to anatomical landmarks. Mucocutaneous flap is put in place, the obtained aesthetic result is visually assessed and sutured. Air-conducting intranasal splints are inserted into the nasal cavity and fixed with one interrupted suture, which are removed on an outpatient basis on the third day. Adhesive bandage of strips is applied on the nose, an external thermoplastic nasal splint is fixed, which is removed on an outpatient basis on 14th day.

EFFECT: method enables achieving a stable and uniform narrowing of the osteocartilaginous portion of the middle arch of the nasal arch in structural rhinoplasty, fixing the short nasal bones without injuring the soft tissues of the nasal covering, reducing the length of the operation.
