FIELD: medicine.
SUBSTANCE: group of inventions relates to dentistry, namely to orthodontics, and is intended for use in detecting dysfunction of temporomandibular joints in patients and their treatment using aligners. Digital diagnostics is performed. Obtained data of CBCT, TRG, 3D scanning of teeth, MRI and axiography are used to determine functional disorders of temporomandibular joints. Aligner positioning the temporomandibular joints and the position of the lower jaw in the digital protocol in the correct therapeutic position is modelled. Aligner is made in the form of transparent tray made of thermoforming plate based on multilayer polyurethane plastic, with individual occlusal surface and occlusal onlays with holes for occlusal occlusion of teeth. Maximum distance of movement of heads of condylar processes of temporomandibular joints is not more than 1.5 mm. If it is necessary to move to a greater distance, the heads of the temporomandibular joints are first moved in the vertical direction, then distraction is performed in the mesio-distal direction at a distance of not more than 1.0÷1.5 mm. Occlusal onlays on the premolars are simulated taking into account the physiological position of the temporomandibular joints in a digital protocol. With the help of aligners to maintain the position of the lower jaw, distalisation of molars is performed. Then, to achieve absolute retrocontrol of the position of the lower jaw, occlusal onlays are removed from the premolars and the occlusal onlays on molars are simulated in a digital protocol. Final alignment of dental arches is performed with control of movement of roots of teeth. Used aligner is made in the form of a transparent tray from a thermoforming plate based on a multilayer polyurethane plastic with an individual occlusal surface and occlusal onlays with holes for occlusal closure of teeth.
EFFECT: due to the introduction of additional elements in the form of occlusal onlays into the aligner structure, the inventions allow correcting the position of the entire lower jaw and temporomandibular joints (TMJ), accurately holding the position of the lower jaw and controlling the TMJ at all stages of treatment.
