METHOD OF ASSESSING EFFICIENCY OF COMBINED ORGAN-PRESERVING TREATMENT OF PRIMARY INITIAL RETINOBLASTOMA IN CHILDREN Russian patent published in 2013 - IPC A61B3/00 A61B6/03

Abstract RU 2486859 C1

FIELD: medicine.

SUBSTANCE: invention relates to medicine, in particular to ophthalmology and is intended for assessment of efficiency of combined organ-preserving treatment of retinoblastoma in children. For this purpose optic coherent tomography is carried out with estimation of: 1) elevation of tomographic section towards vitreous body, 2) diameter of tumour base, 3) degree of tumour tissue homogeneity, 4) degree of hyper-reflectivity intensity, 5) presence or absence of neuroepithelium detachment in zones adjacent to tumour, 6) presence or absence of tumour tissue between pigment and neuroepithelium, 7) presence or absence of smooth choroidal profile, 8) presence or absence of "shadow effect". 1 degree of tumour tissue homogeneity is stated if 5-6 microcalcinates are present, 2 degree is stated if 3-4 microcalcinates are present, 3 degree - 0-2 microcalcinates. 1 degree reflectivity is stated in case of low-reflectivity, 2 degree reflectivity is moderate reflective one, 3 degree reflectivity is highly-reflective, and in case of formation of chorioretinal scar, by: 1) absence of elevation of tomographic section towards vitreous body, 2) absence of tumour base diameter, 3) absence of tumour tissue homogeneity, 4) 3 degree of hyper-reflectivity intensity, 5) absence of neuroepitheliun detachment in zones adjacent to tumour, 6) absence of tumour tissue between pigment and neuroepithelium, 7) presence of smooth choroidal profile, 8) absence of "shadow effect" or in case of formation of calcinate, characterised by: 1) 500-900 mcm elevation, 2) absence of tumour base diameter, 3) absence of tumour tissue homogeneity, 4) 3 degree of hyper-reflectivity intensity, 5) absence of neuroepithelium detachment in zones adjacent to tumour, 6) absence of tumour tissue between pigment and neuroepithelium, 7) absence of smooth choroidal profile, 8) presence of "shadow effect", treatment is estimated as efficient. In case of presence of residual tumor, characterised by: 1) 200-600 mcm elevation, 2) 400-2000 mcm tumour diameter, 3) 2 degree of homogeneity, 4) 2-3 degree of hyper-reflectivity intensity, 5) 1-80 mcm neiroepithelium detachment of zones adjacent to tumour, 6) absence of tumour tissue between pigment and neuroepithelium, 7) presence of smooth choroidal profile, 8) absence of "shadow effect" or in case of latent growth of tumour, characterised by: 1) absence of elevation, 2) absence of tumour diameter, 3) 1 degree of homogeneity, 4) 2 degree of hyper-reflectivity intensity, 5) 1-50 mcm neuroepithelium detachment in case of exophyte growth, 6) presence of tumour tissue between pigment and neuroepithelium, 7) presence of smooth choroidal profile, 8) absence of "shadow effect", treatment is estimated as insufficiently efficient.

EFFECT: method provides possibility of diagnosing minimal signs of tumour resorption, symptoms of residual tumour or signs of latent growth, which makes it possible to select adequate treatment tactics in future, in particular, repetition of transpupillary thermal therapy sessions or realisation of brachytherapy.
