SUBSTANCE: invention refers to medicine. A guide element is provided for perforating an extension of a blind hole in a solid tissue, particularly the jaw bone. A hollow body forms a pressure chamber and has a distal working hole and an inlet for the guide element. The guide element has a through hole, through which a shaft of a working instrument, e.g. a milling tool, can be passed through and introduced into the hollow body. The guide element has a working medium connection for setting an inner pressure in the pressure chamber or in the hollow body. The guide element forms an inner passage, which is closed all around and connects an outlet. The outlet extends into the inlet. The passage extends separately from the through hole along its full length.
EFFECT: higher protection of the device against contamination and reduced risk of the patient's infection.
