FIELD: medicine.

SUBSTANCE: invention relates to ophthalmology. For the surgical management of the complicated fossa of the optic nerve disk (OND), three-port vitrectomy is performed with removal of the posterior hyaloid membrane, removal of an internal boundary membrane (IBM) and formation of an inverted flap covering the OND with a tamponade at the end of the vitreal cavity operation with air. If the neuroepithelial detachment is small - up to 400 mcm in the fovea area and high visual functions, the IBM is removed with a small diameter of 1–1.5 OND; the operation is completed by applying the enriched thrombocyte plasma on the OND. If visual functions are reduced and the height of detachment of the neuroepithelium is more than 400 mcm, a flap on the pedicle is cut in the fovea of the IBM, the flap is turned over and covered with an optic disc pressing the perfluoroorganic compound, operation is completed by application of enriched thrombocyte plasma on OND. If the oedema recurs in the macular area, the avitreal eye with the removed IBM is followed by retinopuncture with drainage from the macular area of the liquid and subsequent introduction of the enriched thrombocyte plasma subretinal and on the OND.

EFFECT: method reduces macular oedema and adherence of the neuroepithelium in the macular area.
