FIELD: medicine.
SUBSTANCE: in neurology, functional and ultrasound diagnostics it can be used to verify a multifocal variant of chronic inflammatory demyelinating polyradiculoneuropathy. Electroneuromyography examines the median and ulnar nerve from both sides on the right and left. The motor fibres of the median nerve are examined when the motor response is removed from m.APB, stimulation is applied to the level of the wrist, elbow, middle third of the shoulder and in the supraclavicular fossa. The motor fibres of the ulnar nerve are examined when the motor response is removed from m.ADM, and stimulation is performed at the level of the wrist, 4 cm distal to the medial epicondyle of the elbow joint, 4 cm proximal to the medial epicondyle of the elbow joint, upper third of the shoulder and in the supraclavicular fossa. The sensory fibres of the median and ulnar nerves on the right and left are also examined when the sensory potential is removed from fingers II and IV, respectively, and stimulation is performed at the level of the wrist. The presence of a motor conduction block (CB) is assessed. The identified characteristics are estimated in points. An ultrasound examination of the median and ulnar nerves is performed in real time from both sides in the transverse plane at the level of the middle third of the forearm and the middle third of the shoulder, as well as the trunks of the brachial plexus from both sides in the interscalene space in the transverse plane. The values of the cross-sectional area of the nerve trunks and brachial plexuses are estimated in points at each of the above points studied. All points are summed up. If the score is 5 or higher, a multifocal variant of chronic inflammatory demyelinating polyradiculoneuropathy is diagnosed.
EFFECT: method allows to easily, simply, non-invasively, and reliably diagnose a multifocal variant of chronic inflammatory demyelinating polyradiculoneuropathy by evaluating a complex of the most significant indicators.
